QT prolongation and arrhythmia
Ibogaine has been associated with prolongation of the QT interval, an ECG measurement that can signal increased vulnerability to dangerous rhythm disturbances. The published review of ibogaine-related fatalities documents why cardiac history, concurrent substances, and medical circumstances cannot be treated as afterthoughts. Alcohol, medicines, and metabolic conditions
Alcohol use can be linked to electrolyte abnormalities, liver-related concerns, withdrawal complications, and medication changes. These factors may be relevant to cardiac and neurologic risk. Interactions must also be considered with medicines commonly used in alcohol use disorder, as well as medicines affecting heart rhythm, seizure threshold, sedation, or metabolism. General lists cannot establish an individual interaction profile.
Neuropsychiatric effects
Ibogaine can involve intense altered states, confusion, impaired coordination, sleep disruption, anxiety, and distress. A history of psychiatric symptoms, neurological conditions, or substance-related instability may change the risk picture. Evidence does not support treating a powerful subjective experience as proof of safety or benefit.
